Forms and Contracts

Please be aware of pirating. The Academy does not sell its attendee list to third party marketing companies. Any vendor selling a list of those registered to attend the Academy Annual Meeting should be ignored.

Exhibitors Needing 10+ Rooms

Exhibitors requiring 10 or more rooms must complete the Sub-Block Application


To finalize your block, the Hyatt Regency New Orleans must receive the individual guest names by January 14, 2018. The Hyatt Regency will release unused rooms in your block. (An unused room is one that does not have the name of a registered attendee attached to it.)


Once your block has been approved you will receive an email confirmation with booking instructions on how to make your hotel reservations.

Changes, Cancellations, and Refunds

The last day to cancel rooms without penalty is January 18, 2018. Effective January 19, 2018 a $30 per room processing fee will apply for cancellations. Additionally, the hotel will charge one night's room and tax for rooms that are cancelled from January 20 – February 14, and charge for all nights beginning on February 15.
